A North Liberty woman who formerly worked in a Wellman nursing home is facing prescription drug violation and dependent adult abuse charges. 

Records show that 43-year old Rachel Flowers had been charged in October of 2025 with prescription drug violation and dependent adult abuse, both serious misdemeanors. 

According to court documents, an investigation by the Iowa Department of Inspections, Appeals and Licensing, Medicaid Fraud Control Unit, found that Flowers was working as a nurse in Marion where she was a caretaker for a dependent adult. The patient told the investigator that he believed that he and Flowers were in a relationship. While the defendant was a caretaker she allegedly took prescription medication from the adult. Flowers left the Marion facility. 

Flowers was later investigated at another facility in which she worked for missing narcotics. During the investigation, it was reported that she had in her possession several stock medications and other supplies she had taken from that facility and the prescription medications that were taken from her patient from Marion. 

A jury trial has been scheduled for March 30th in Linn County. 

Previously Flowers had plead guilty to fifth-degree theft charges. Court records show that in April of 2025, the Washington County Sheriff’s Office responded to a call of a theft at a nursing home in Wellman. The director of nursing had contacted authorities stating that Flowers had taken property belonging to the business. The total amount of products that were taken amounted to $653.66. Flowers received a deferred judgment of one year unsupervised probation.  

Flowers’ license status has now been changed to “suspended” by the Iowa Board of Nursing, while that court case is ongoing.
