Funeral services for 51 year old Charles “Chuck” Lewis of Burlington, formerly of Ottumwa and Washington will be held at 10:30 Monday at the Linby Baptist Church in rural Packwood.  Burial will be in the Mt. Zion Cemetery near Martinsburg.  Visitation will begin at 2:00 Sunday with the family present from 3:00 to 4:30.  A general memorial has been established.  The Gould Funeral home is assisting with the arrangements.
