A Kalona nursing home is facing $3,000 in fines. According to an investigation by the Iowa Department of Inspections and Appeals, a resident at Pleasantview Home received 10 times the intended dose of the medication Plendil for six and a half weeks this spring, and was discovered after the resident was transported to a hospital. The citation also lists two falls by residents when staff did not follow protocol.

Pleasantview Home Executive Director David Heusinkveld stated that the facility self-reported the issue with dispensing medication immediately to the state and that there has been a “monumental effort” to correct the issues in full cooperation with the state.
